HCS acknowledges that medical record and billing information is highly confidential and agrees not to disclose or use information for any purpose except as permitted or required to perform necessary services to HCS clients or as required by law.
HCS maintains compliance with federal regulations (Health Insurance Portability and Accountability Act, 2003) by insuring the following:
Agree not to use or further disclose Protected Health Information (PHI), other than as required by job responsibilities. PHI includes the following: patient name, address, social security number, age, telephone number, diagnosis, tests, procedures, condition, and any other personal information.
HCS agrees to use appropriate safeguards to prevent any use or disclosure of PHI
HCS will immediately report any unauthorized disclosures to the client.
